COVID Vaccination Passport/Certification

Please do not contact the practice to request certification/evidence of COVID vaccination status.

Evidence of COVID-19 Vaccine Status can be found here: NHS Inform - COVID-19 vaccination status

Until further notice all non-urgent appointments in the practice, for example annual reviews, certificates, reports etc have been scaled back.  We hope by doing this we can reduce the spread of infection, keep our patients and staff safe thus keeping our frontline services operational for those patients who need us most.  This measure will not prevent you accessing the care you need - we hope to deal with your contacts by telephone and video where appropriate and will make safe provision should we need to see you urgently.  We thank you for your patience and understanding during this unprecedented situation.

If you require medical attention, we are currently operating telephone triage appointments whereby a member of our clinical team will call you back in order to asses your needs.  Please assist us by giving staff as much details as possible - this will allow them to signpost you to the most appropriate person.

If, following triage, you are invited to attend the surgery we request that you follow our social distancing measures.  Staff will direct you to a designated waiting area (this may not be the usual area).  You will be unable to wait in the reception or other public areas as this will prevent a safe flow of traffic for staff and other patients attending.  If it is safe for you to do so, we would request that you attend alone or with the assistance of a maximum of one individual if this is required.  Doing this will help us reduce any risks, given that our waiting areas can only accommodate limited numbers at this time. Please wear a face covering if attending.  Facilities are available to sanitise your hands upon arrival and leaving.

To ensure our telephone lines are accessible to those who with an urgent clinical need, we will no longer be accepting repeat prescriptions via the telephone.  Doing this over the last few months helped patients obtain their medication without the need to attend the surgery, pharmacy, post boxes etc whilst helping reduce the spread of infection.  This next phase however means we must prioritise our resources to those who need to contact us with medical problems.

We respectfully request you return to your usual method of ordering or try either the online or email system.  If you do not wish to opt for either of these, an external post box is available to drop off requests.  Our internal postbox is not currently in use and we request that you do not return to the surgery to collect your prescription, instead please choose a pharmacy convenient to you.

We request you wear a mask or face covering if you have been invited to attend the practice.  If you are exempt from this, we would appreciate you wearing a loose scarf over your mouth and nose.

You may normally attend our nursing team for review of certain conditions. Whilst we have resumed the majority of these,  the review you are used to may be carried out differently. If you have any queries, please contact the practice.  We thank you for your patience.
